Health Forms

To ensure the safety and well-being of all students, the school has specific procedures regarding the administration of medication during school hours. As a general principle, we aim to avoid administering medication at school whenever possible.

 

The three forms below apply to both short-term medication (e.g. antibiotics) and long-term medication. If medication must be administered at school, the relevant form must be completed, printed, signed, and handed in to the school office before the medication can be given. Please note that emergency medication requires an additional procedure and must also be signed by the treating physician.

General information about a student's health (allergies etc) needs to be provided through Vigilo.

Duplicate Information Request (Step-parents)

In order for step-parents to be informed about the child’s progress and well-being - including potential participation in parent-teacher conferences - a duplicate information request must be signed by all parents with legal custody.​

Applications for leave

Class Teachers can grant a leave for up to two days. For absences longer than two days, an application must be sent to the Principal prior to the leave through the Vigilo Portal. We do not encourage families to take holidays during term time as this is disruptive to a child’s education.
